IMPORTANT NOTICE FOR APPLICANTS

Please be advised that all new applicants seeking registration are now required to complete and return a Release and Discharge Relating to Consent to Disclosure of Criminal Record Information form.

This form is issued by the Ontario Provincial Police and, once completed, authorizes the OPP to release the results of your criminal background search to the Ontario Motor Vehicle Industry Council (OMVIC).

Criminal background searches are required and conducted for all individuals seeking registration under the Motor Vehicle Dealers Act. Your application will be considered incomplete if you fail to return this form.
